Application for New Membership
Students wishing to apply for student membership may submit a completed application form along with a membership fee of $500.
Registered chiropractors wishing to join the CDAHK are required to prepare the following documents:

1. A completed membership application form |

2. A completed insurance application form (if you opt for professional indemnity coverage with the CDAHK) |

3. A certified true copy of your graduation certificate/diploma issued by a CCE approved Chiropractic College/University |

4. A certified true copy of the Registration Certificate and a current Practicing Certificate issued by the Chiropractors Council of Hong Kong (for Hong Kong memberships) or an overseas regulating body (for overseas memberships) |

5. A cheque payable to the “Chiropractic Doctors’ Association of Hong Kong Limited” (for Hong Kong memberships) or pay online by “PayPal” |

- Membership fee is non-refundable.
- The cost of membership is inclusive of your professional indemnity coverage. The coverage is under a group policy of CDAHK. The insurance coverage is provided by QBE HongKong & Shanghai Insurance Ltd.
